Printing Information:

HTML:  With the form on the screen, somewhere on the form, right click and select print.  This will print the form only and not the entire webpage.

PDF:  Use the PDF print function at the top of form.  This will print the form only and not the entire webpage.

DOC:  Use the Word print function at the top of form.  This will print the form only and not the entire webpage.
