Note:  Your primary source of tournament entry forms should be at your golf course.  In the event you cannot obtain a preprinted form at your course, these forms may suffice.  Due to differences in browsers and printers, we do not guarantee that they will accurately duplicate the preprinted forms.  Ugly forms are acceptable as long as they contain all necessary information.

You cannot submit the entry form online.  Depending on your browser, you may be able to fill in the form on-screen and then print the form >fill & print< ; but the printed form must then be submitted in person or by mail.  Most PDF and Word entry forms are fill able on-screen.
